The post-hospital discharge phase is a critical yet vulnerable time for patients, where poor management can lead to over $53 billion in preventable readmission costs annually. Kouper tackles this issue with AI-driven tools that enhance patient engagement, automate scheduling, and collect data across emergency, inpatient, and urgent care transitions.
